Your annual biometric screening is a way to say you care about you health. It will automatically earn you between 2,000 - 4,000 points into your Go365 account.
For individuals who were unable to attend an onsite biometric, the screening can be done at your Primary Care Physician or at a number of other locations. Click here for information on how to contact a provider. This form will need to be completed by the provider.

For your Information:
Allow up to 7 business days from the date of your screening to access your report on-line.
E-mail myhealth@hooperholmes.com if you need your barcode to access your report on-line or for other questions.
A document is being mailed to your home with the results of your biometric screening
Your results/Points will automatically load to Go365 within 14 business days of last screening (3.15). You do NOT need to upload the results yourself.
